Consent to treat beginning
2013-14 school year
Warren Township Schools have partnered with Community Health Network as of January 2013. As part of this new partnership, the health clinics at each school will be staffed with a Community Health Network Nurse.
Beginning at the start of the 2013-2014 school year, ALL students must have a signed consent each year for Community Health Network. This signed consent allows for the care and treatment of our students to be provided by the Community Health Network Nursing staff.
